Say This:

I would say, "I appeciate everything you have done for me. But we aren't really on the same wavelength, so I am going to look for another doctor."

It's unlikely that you can turn a rude doctor into a caring one, no matter what you say. And why continue to go to someone who makes you uneasy? (Spoken as someone who has put up with a remarkably rude doctor for a year without doing anything about it.)
