I'm pretty sure he has a war injury that causes that stiff posture.

The Senator is after all 72, and has had numerous injuries in the service. He did lose five fighter jets in his navel service, not to mention his time in captivity and bouts with skin cancer. When he crashed in North VietNam, Senator McCain suffered a number of broken bones, none of which, of course, were set properly. Furthermore, he was often forced to stand with his wrists tied tightly together in front of him, his upper arms pulled back and tied tightly behind him, and a rope around the back of his neck tied tightly to a rope around his waist, causing his head to be bent forward.
